Responsibilities Provide direct patient care during overnight shifts, including assistance with activities of daily living such as bathing, dressing, and mobility support using equipment like Hoyer lifts. Monitor and record vital signs, observe changes in patient condition, and communicate findings promptly to nursing staff. Assist with medication administration following proper procedures and maintaining accurate medical documentation within Electronic Health Record (EHR) systems. Support patients with specialized needs including dementia, Alzheimer's, memory care, behavioral health, or those requiring hospice or palliative care. Maintain aseptic techniques during procedures such as catheter care and infection control practices aligned with HIPAA regulations. Operate medical equipment safely, including phlebotomy tools when necessary, and ensure all medical records are accurately updated. Collaborate with interdisciplinary teams in acute care, ICU, emergency medicine, or long-term care environments to deliver comprehensive patient service. Skills Proven experience as a CNA with knowledge of medical terminology and physiology relevant to senior and pediatric care. Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for medical documentation. Experience working in diverse healthcare settings including hospitals (Level I Trauma Centers), nursing homes, assisted living facilities, or outpatient clinics. Strong understanding of infection control practices, aseptic technique, and basic life support (BLS). Ability to work independently during overnight shifts while maintaining high standards of patient safety and comfort. Knowledge of specialized care needs such as dementia/dementia care, behavioral health management, or post-acute/sub-acute care environments. Excellent communication skills to effectively document patient information and coordinate with healthcare teams across disciplines like geriatrics, pediatrics, or home & community care.
