In this vital role, you will provide high-quality patient in an Adult Medical Day Center. Your primary focus will be supporting individuals with diverse needs, including those with developmental disabilities, dementia, Alzheimer's, and hospice care requirements. This position offers flexibility and the opportunity to make a meaningful difference in the lives of our clients while working in a dynamic and supportive environment. Duties Observe and monitor patients' conditions, recording vital signs and changes in health status accurately. Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies. Serve meals and assist with feeding to ensure nutritional needs are met, considering dietary restrictions for specific health conditions. Maintain cleanliness through light housekeeping duties. Document all patient interactions and medical information thoroughly in medical records or documentation systems to ensure continuity of care. Requirements Valid CNA or CHHA certification with current licensure in good standing. Proven experience working in assisted living facilities, day centers, or home health environments; experience with long-term care is highly valued. Knowledge of medical terminology, medication administration protocols, and vital signs measurement. Familiarity with accurate documentation of patient care activities. Ability to work independently and as part of a multidisciplinary team supporting individuals with disabilities or chronic health conditions.
