The ideal candidate will have experience working with diverse populations, including seniors, individuals with developmental disabilities, and those requiring memory or Alzheimer's care. This role offers an opportunity to make a meaningful difference in patients' lives through attentive caregiving and support. 3 Years experience or more encouraged Duties Provide direct patient care, including assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility assistance Monitor and observe patient conditions, documenting vital signs and changes in health status accurately using EMR/EHR systems Assist with medication reminder's following proper protocols and maintaining compliance with HIPAA regulations Support patients with mobility aids such as Hoyer lifts and assist with transfers safely Support patients with dementia or Alzheimer's by implementing memory care strategies and ensuring a safe environment Assist with meal preparation and feeding as needed, ensuring dietary restrictions are followed Perform light housekeeping duties including laundry and cleaning to maintain a safe and hygienic environment Document all patient interactions thoroughly in medical records and ensure confidentiality at all times Collaborate with healthcare teams to coordinate patient care plans and communicate any concerns or changes Qualifications Valid CNA or CHHA certification required; additional certifications such as CPR or first aid are preferred Experience working in nursing homes, assisted living facilities, hospice care, or home health environments is highly desirable Knowledge of medical terminology and familiarity with EMR/EHR systems is beneficial Strong understanding of patient observation techniques, medication administration, and infection control protocols (including HIPAA compliance) Experience working with individuals with disabilities, dementia, Alzheimer's disease, or in memory care is advantageous Ability to work compassionately with diverse populations including seniors and those with developmental disabilities Excellent communication skills and the ability to work effectively within a team environment Must be reliable, detail-oriented, and committed to providing high-quality patient care This position offers an enriching opportunity for caregivers passionate about improving the quality of life for their patients through dedicated service.
