In this vital role, you will provide essential patient care and support to individuals across various care settings, including hospitals, long-term care facilities, assisted living communities, and home care environments. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately within electronic health record (EHR) systems such as Epic or Cerner. Support medication administration under supervision, ensuring compliance with HIPAA regulations and aseptic techniques to prevent infection. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Collaborate with nursing staff to deliver comprehensive patient care in diverse settings such as ICU, emergency departments, pediatrics, geriatrics, behavioral health units, and post-acute care environments. Maintain detailed medical documentation and ensure all patient records are accurate and up-to-date in medical records systems like eClinicalWorks or Meditech. Assist with procedures requiring sterile technique or invasive interventions such as catheter care or phlebotomy when trained and authorized. Support patients with complex needs including infectious disease management, trauma care in Level I or Level II trauma centers, and hospice services. Promote patient safety by adhering to protocols for vital signs monitoring, anatomy knowledge application, and emergency response procedures in urgent or acute care situations. Experience Prior experience working as a CNA in hospital medicine, medical-surgical units, outpatient clinics, or long-term care facilities is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate documentation. Experience providing specialized dementia or Alzheimer's care within memory or behavioral health units is advantageous. Knowledge of physiology and anatomy relevant to patient assessments across diverse age groups including pediatrics and geriatrics. Hands-on experience with medical procedures like medication administration, catheter care, vital signs measurement, and basic life support certification is required. Familiarity working within high-acuity environments such as ICU settings or Level I trauma centers enhances your candidacy. Ability to work effectively with individuals with disabilities or developmental challenges in home & community care settings is a plus.
