Our team works closely with patients and families in the home setting, offering support, guidance, and personalized care during challenging times. Position Overview We are seeking a compassionate and dependable Palliative CNA to join our growing team. In this role, you will provide personal care and supportive services to patients in the home under the supervision of a Registered Nurse or therapist. The ideal candidate is caring, reliable, and passionate about helping patients maintain comfort and dignity in their own homes. Key Responsibilities Assist patients with personal care, including bathing, grooming, hygiene, dressing, mobility, and transfers. Follow written care instructions developed by the RN or therapist. Assist with exercises, ambulation, range-of-motion activities, and therapy-related support. Support medication self-administration as permitted by state regulations and agency policy. Observe and report changes in patient condition to the RN or Supervisor. Maintain accurate documentation of care and patient condition. Provide household services essential to patient care and comfort. Build positive, professional relationships with patients, families, and team members. Participate in patient care conferences and interdisciplinary team communication.
Qualifications Required:
High school diploma or GED. Successful completion of a state-approved nurse aide training and competency evaluation program. Active CNA certification in good standing. Valid driver's license and proof of current automobile insurance. Reliable transportation. Ability to participate in on-call rotation. Not listed on the OIG Excluded Provider List.
Preferred:
1 year of CNA, home care, hospice, or palliative care experience. What We're Looking For Compassionate attitude toward caring for patients with serious illness.
