JOB DESCRIPTION

Job Summary Deliver compassionate, patient-centered care that provides dignity, support, and care to patients and families. Job Specific Duties Provide personal care services to patients in accordance with the plan of care and under the supervision of a licensed nurse Assist patients with activities of daily living including bathing, grooming, dressing, mobility, toileting, and feeding Measure and record vital signs including temperature, pulse, respiration, and blood pressure as directed Observe and report changes in patient condition, safety concerns, or care needs promptly to supervising staff Assist with ambulation, transfers, and positioning to promote comfort and prevent complications Maintain accurate documentation of care provided in the EMR in accordance with agency policies Support infection control, safety, and risk management procedures in the care environment Provide emotional support and companionship to patients and families Participate in in-service training and continuing education programs as assigned
Other Duties:
Provide care and services in accordance with infection control, safety, and risk prevention protocols Accurately document clinical interventions, care coordination, and communication in a timely manner Participate in interdisciplinary meetings, case reviews, and continuing education opportunities Maintain compliance with HIPAA and all company privacy and confidentiality standards Represent the organization positively and professionally in patients' homes and community settings Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities Provide courteous, timely, and professional customer service to patients, families, and partners Exhibit adaptability to changing responsibilities and flexibility in completing assignments Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook Communicate and interact professionally and respectfully with others to support team goals Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company Work Environment Primarily works in the field Supervisor Clinical Manager - Home Health or Clinical Manager - Hospice Supervises None Risk Level Patient care or physical work with injury risk
Requirements:
Qualifications High school diploma or equivalent required Completion of a state-approved Nurse Aide Training program Current certification as a Nurse Aide in the state of practice At least one year of experience preferred; home health or hospice experience a plus Ability to follow instructions, provide safe and compassionate care, and document services accurately Strong communication and interpersonal skills with patients, families, and staff Reliable transportation, valid driver's license, and auto insurance for home visits Physical Requirements Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
