Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using tools like Hoyer lifts and other assistive devices. Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature, accurately documenting all findings in electronic health records (EHR) systems. Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations and aseptic techniques. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Collaborate with multidisciplinary teams in long-term care, hospice, and respite care to deliver comprehensive patient service. Maintain infection control standards by practicing proper aseptic techniques and adhering to safety protocols in infectious disease or skilled nursing environments. Support patients with developmental disabilities or behavioral health needs by providing compassionate caregiving tailored to individual requirements. Ensure medical documentation is complete and accurate while maintaining confidentiality of medical records in compliance with HIPAA regulations. Assist in patient transfers using Hoyer lifts or other mobility aids safely and effectively across inpatient, outpatient, or home & community care settings. Skills Proven experience working with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation. Demonstrated knowledge of physiology, anatomy, medical terminology, and vital signs monitoring essential for patient care. Critical care experience including ICU or Level II trauma center exposure; familiarity with acute care procedures is highly valued. Experience supporting diverse populations such as seniors in geriatrics or assisted living facilities; familiarity with dementia and memory care is preferred. Skilled in medication administration, catheter care, aseptic technique, and infection control practices across various healthcare environments. Strong caregiving skills combined with excellent communication abilities to collaborate seamlessly within multidisciplinary teams. Knowledge of medical-surgical nursing practices and familiarity with EMR/EHR systems like Home & Community Care software or DME documentation tools. Experience working with people with disabilities or behavioral health needs; understanding of trauma-informed care approaches is advantageous. Ability to adapt quickly in fast-paced environments such as urgent care centers or hospital medicine units while maintaining high standards of patient service.
