Vaccination Required:

Flu shot, COVID proofs (or declination), and 2 negative TB tests or 1 Quantiferon;

Varicella Licensure Requirements:

Providers will not receive a hospital application until they have a full CRNA license, and no temporary privileges are being granted on-site.
