Transitional Care Unit Position Summary Summary:

Utilizing Excellent Customer Service skills, The CNA assists the Registered Nurse in delivery of high-quality patient care in the Transitional Care Unit. Collects and records clinical patient data, obtains specimens. Takes vital signs and assists with Activities of Daily Living (ADLs). Documents patient activity in EMR.

Qualifications Responsibilities:

Education Required Must complete in-house orientation program High School Diploma or equivalent

AND:

Successful completion of an approved 120-hour Nurse Aide training program Demonstrate sensitivity to the population served. Experience And Skills Excellent interpersonal skills Speaks, reads and writes English Minimum of 1 year related experience preferred. Technical skills required for patient care. Good verbal and written communication skills. License/Certifications Certified as a Nursing Home Nurse Aide by the NYS DOH (CNA).
