Nursing Home Primary Care Location:
Stamford, NY (Catskills Region)
Schedule:
Monday-Friday Salary:
$130,000
  • $150,000 + RVU Bonus Opportunities A growing medical group providing primary care services to nursing home patients is seeking an experienced Physician Assistant to join their team in a skilled nursing facility located in the Stamford area of New York's Catskills Region.
This role focuses on providing high-quality primary care to geriatric patients within a nursing home environment. The provider will manage a panel of patients, coordinate care with facility staff, and ensure patients receive comprehensive medical attention. Schedule The provider will work Monday through Friday with flexible scheduling. Providers may choose their daily working hours (for example 8-3, 9-5, or similar). Providers must remain available and accessible by phone between 8:00 AM and 6:00 PM for patient care needs. Compensation Structure The group offers flexible compensation models including: Base salary between $130,000
  • $150,000 Additional RVU-based productivity bonuses paid quarterly Option for fully RVU-based compensation for higher earning potential Option to work as either W2 or 1099 Benefits Health insurance (single plan) Dental and vision coverage Malpractice coverage DEA reimbursement Four weeks PTO Clinical Responsibilities Provide primary care services to geriatric patients in a nursing home Perform routine visits, follow-ups, and chronic disease management Coordinate care with nursing staff and facility administration Participate in care management programs including CCM and RPM Document patient encounters in facility EMR systems (SigmaCare or PCC) Requirements Active New York Physician Assistant license Active Medicare and Medicaid provider ID Experience working with geriatric patients preferred Skilled Nursing Facility experience preferred Prior experience in nursing homes or long-term care settings is highly valued Physician Assistant (PA)•Nursing Home Primary Care Oneonta, NY 13820 $130,000•$150,000 a year•Full-time $130,000•$150,000 a year•Full-time Physician Assistant (PA)•
