Job Description

ICM TEAM LEAD - RN FULL TIME / 8A - 5P / 80 HOURS PER PAY
The ICM Unit Team Leader is a registered professional nurse with advanced knowledge, critical thinking skills and leadership experience in case management, change management, performance improvement, conflict resolution and information management. The Team Leader provides direct supervision for the overall daily operations of the specific unit/team that includes but is not limited to, technical responsibilities, staffing support, utilization review, care coordination and team facilitation. This person is knowledgeable about third party payer reimbursement which impacts healthcare delivery systems. The Team Leader also provides guidance and follow up to the units' case managers, especially in cases with complex issues. The role is primarily focused on the continuum of care needs of our patients and their families. Close coordination and daily collaboration with the ICM Managers, Medical Director, physicians and other members of the healthcare team is critical. Consistent coaching and mentoring the staff to achieve optimal patient care outcomes is a function of the ICM Team Leader. As a role model, effective communication skills and professionalism are critical. The Team Leader assists the ICM Managers with the identification of opportunities for improvement, case management and social services needs and utilizes research, analysis and evidence based practice.
Education:
A Bachelor's degree in Nursing is required. Masters in Nursing or healthcare related field preferred.
Licensure:
Current Ohio RN License Experience:
A minimum of 3-5 years' experience in a clinical/acute care setting with back ground in Case Management, Utilization Management, Performance Improvement and Discharge Planning. Must have past experience with managing people in a fast-paced environment. Preferably 3-4 years of management experience. Demonstrated expertise in effective communication, team building and consensus management required. Experience with information systems and report analysis required.
