Job Overview We are seeking a compassionate and dedicated Home Health Aide to provide essential support to patients in their homes. The ideal candidate will play a crucial role in assisting individuals with daily activities, ensuring their comfort and well-being. This position requires a strong commitment to patient care, as well as the ability to work independently and as part of a team. Duties Assist patients with personal care tasks such as bathing, grooming, and dressing. Help with mobility and transfers, ensuring patient safety at all times. Monitor and record vital signs, including temperature, pulse, and respiration rates. Provide companionship and emotional support to patients and their families. Administer medications as prescribed and assist with medical equipment usage. Maintain accurate medical records and report any changes in patient conditions to healthcare professionals. Collaborate with multidisciplinary teams including clinic staff, hospital medicine teams, and behavioral health specialists to ensure comprehensive care. Support patients with specific needs such as pediatrics or infectious disease care when applicable. Assist with phlebotomy tasks if qualified, ensuring adherence to safety protocols in a Level II Trauma Center environment. Requirements High school diploma or equivalent; certification in home health aide training preferred. Previous experience in acute care or hospital settings is an advantage. Knowledge of medical records management is beneficial. Ability to work effectively in various healthcare environments including clinics, OR settings, and behavioral health facilities. Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams. Physical stamina to perform tasks that require standing for long periods and lifting up to 50 pounds when necessary. A valid driver's license may be required for travel between patient locations.
