Full Job Description Job Summary:
The Activities Coordinator is responsible for planning, coordinating, and implementing engaging recreational, social, and community integration activities for residents across five buildings within the ICF-IID facility. This role includes organizing and leading weekly outings for each building, providing in-building support after outings, and working closely with residents on active daily living skills development to promote independence and quality of life.
Key Responsibilities:
Program Development & Implementation
  • Plan and lead weekly outings for all five residential buildings (e.g., parks, museums, events, restaurants, etc.).
  • Coordinate transportation and staffing needs for outings, ensuring safety and inclusivity.
  • Develop and implement in-house activities for residents who do not attend outings.
  • Support individualized programming based on resident needs, preferences, and Person-Centered Plans (PCPs).
  • Plans and facilitates holiday events with residents.
  • Will provide a weekly calendar a week prior for approval by QIDP. Resident Engagement
  • Engage with residents before and after outings to ensure comfort, preparedness, and feedback.
  • Remain in the building post-outing to support residents with transitioning back, processing experiences, and resuming daily routines.
  • Work directly with residents on active daily living skills such as hygiene, social skills, basic cooking, money management, and communication. Collaboration & Communication
  • Coordinate with Direct Support Professionals (DSPs), Case Managers, and other interdisciplinary team members.
  • Attend care plan meetings and contribute activity-related progress and observations. Documentation & Compliance
  • Maintain accurate records of activities, attendance, goals, and outcomes.
  • Ensure all activities comply with ICF-IID regulations, safety standards, and residents' rights.
  • Submit monthly activity calendars and outing schedules for approval.
  • Qualifications:
  • High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Recreation, or related field preferred.
  • Minimum 1-2 years of experience working with individuals with intellectual and intellectual disabilities (IID).
  • Experience planning group activities and community outings.
  • Strong interpersonal and communication skills.
  • CPR/First Aid certification (or willingness to obtain).
  • Valid driver's license with clean driving record; ability to drive facility vehicle for outings.
  • Work Environment:
  • Indoor and outdoor settings, including community venues and in-home environments.
  • Requires physical ability to assist with resident mobility and transportation.
  • Flexible schedule, including availability for evening and weekend activities.
