Responsibilities:
- Provide personal care and assistance with daily living activities to residents, including bathing, dressing, grooming, and toileting
- Assist with transfers, ambulation, and mobility exercises as needed
- Monitor and report changes in resident condition to licensed nursing staff
- Maintain accurate and timely records of resident care and services provided
