Job Description

Certified Nursing Assistant (CNA) UPMC Northwest Hospital Transitional Care Unit Join our 16 bed Transitional Care Unit , providing short term skilled nursing and strengthening care to help patients safely transition back to the community following acute illness. Responsibilities
  • Provide direct resident care under the direction of the RN and LPN
  • Assist with activities of daily living including bathing, dressing, feeding, toileting, ambulation, and transport
  • Respond promptly to resident needs and call lights
  • Perform routine tasks such as vital signs, weights, and intake and output
  • Assist with feeding, deliver nourishments, and fill water pitchers
  • Observe and report changes in resident condition to licensed nursing staff Safety and Clinical Support
  • Apply, monitor, and remove restraints per physician and facility protocols
  • Use diversional activities to reduce or prevent restraint use
  • Document care accurately in the medical record
  • Follow all safety, infection control, and confidentiality policies
  • Respond to emergent situations within the scope of the nursing assistant role
  • Maintain current CPR certification Unit and Team Support
  • Maintain a clean, organized, and safe environment
  • Assist with linen changes, equipment cleaning, and room organization
  • Stock supplies and report equipment malfunctions
  • Support unit operations including call lights, phones, meal trays, and transport
  • Communicate effectively and maintain professional relationships
  • Accept feedback and actively work to improve skills and performance
