Job Description

Job Summary:
Are you ready to lead, mentor, and inspire in a dynamic elder care setting? We're seeking a Lead CNA for our PACE Day Center, reporting to the Day Center Director. In this pivotal role, you'll organize daily CNA assignments, provide ongoing support and training for new and established staff, and help maintain a calm, positive environment for our participants. You'll serve as a vital link between the interdisciplinary team (IDT), sharing information from meetings and ensuring that staff and participants benefit from coordinated, compassionate care. As the Lead CNA, you'll work closely with both the Day Center RN and the Health Center, collaborating to address participant concerns and that nursing recommendations are carried out. You'll oversee completion of all CNA tasks, monitor vital signs and documentation, enforce behavioral and infection control protocols, and ensure care plans are followed accurately. Your leadership extends to facilitating staff education, guiding competency assessments, assisting with interviews, and solving staffing challenges across departments.
