Responsibilities Provide personal care as outlined in the Hospice Aide Plan of Care. Document all care provided accurately and in compliance with agency policies. Report any deviations or changes in the Plan of Care to the Case Manager or designated staff promptly. Safely utilize and operate durable medical equipment as required. Foster teamwork by understanding the roles of other care team members and actively participating in team and staff conferences. Assist with ambulation, transfers, and range of motion exercises as documented in the Hospice Aide Plan of Care. Offer respite care for the patient's family or caregiver during on-site visits when appropriate. Actively participate in Quality Assurance and Performance Improvement (QAPI) activities and attend Interdisciplinary Group (IDG) meetings as appropriate. Qualifications Completion of a minimum of 75 hours of training, including at least 16 hours of supervised practical training and 16 hours of classroom instruction. At least one year of experience as a CNA, hospice aide, or home health aide (hospice experience preferred). Must be at least 18 years old. Valid driver's license with access to an insured, reliable automobile in good working condition. Strong verbal and written communication skills. Job Environment The ability to access patients' homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care. Physical activities will include walking, sitting, stooping, standing, and performing minimal to maximum lifting and turning of patients.
