Job Description

Job Summary We are seeking a compassionate and dedicated Certified Nurse Aide (CNA) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care, assist with daily living activities, and support residents across various care settings including assisted living, nursing homes, and home health environments. Your expertise will help enhance the well-being and comfort of individuals with diverse needs, including those with developmental disabilities, dementia, Alzheimer's disease, and other memory care requirements. This position offers an opportunity to make a meaningful difference in the lives of our patients while working in a supportive and engaging environment. Responsibilities Observe and monitor patients' conditions regularly, accurately documenting vital signs, medical records, and changes in health status using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility support utilizing equipment such as Hoyer lifts when necessary. Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality. Perform specialized care procedures such as catheter care, hospice support, dementia and Alzheimer's care, and memory care activities tailored to individual needs. Support patients during medical procedures by assisting with medical documentation and ensuring accurate recording of vital signs and other clinical data. Provide compassionate assistance with meal preparation and feeding, ensuring nutritional needs are met in accordance with dietary guidelines. Maintain a clean and safe environment by assisting with laundry, cleaning duties, and ensuring infection control standards are upheld at all times. Collaborate effectively with healthcare teams to deliver comprehensive patient care and communicate any concerns or observations promptly. Support patients' emotional well-being through attentive caregiving and respectful communication, especially when working with individuals with disabilities or cognitive impairments. Experience Prior experience working as a Certified Nurse Aide in nursing homes, assisted living facilities, home health agencies or similar settings is highly preferred. Demonstrated proficiency in patient observation, vital signs measurement, medication administration, and medical terminology. Experience working with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease is advantageous. Familiarity with EMR/EHR systems for medical documentation is essential. Knowledge of hospice care practices, catheter care procedures, Hoyer lift operation, and long-term care protocols is desirable. Ability to handle sensitive situations with compassion while maintaining confidentiality under HIPAA guidelines. Valid CPR certification is required; additional certifications such as dementia care training or medication administration certification are a plus.
