Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts when necessary Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately in electronic health record (EHR) systems such as Epic or Cerner Support patient care plans by providing medication reminders and ensuring adherence to prescribed treatments under supervision Maintain a safe environment by following aseptic techniques, infection control protocols, and proper catheter care procedures Document patient interactions thoroughly in medical records while respecting HIPAA regulations for confidentiality Collaborate with healthcare teams in acute care, ICU, emergency medicine, or behavioral health settings to deliver comprehensive patient support Assist with specialized care needs including Alzheimer's/dementia/memory care, pediatric or geriatrics populations, and individuals with developmental disabilities or behavioral health conditions Skills Proven experience working with diverse patient populations across settings such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or home care environments Familiarity with EMR/EHR systems like Epic, Cerner, eClinicalWorks or similar platforms for medical documentation and patient records management Knowledge of medical terminology, physiology, anatomy, and basic life support techniques essential for effective patient service Experience with advanced caregiving equipment including Hoyer lifts and other mobility aids; proficiency in catheter care and vital signs measurement Ability to perform phlebotomy procedures and assist with medication administration when certified or trained accordingly Strong understanding of infection control practices including aseptic technique and handling infectious diseases in various clinical settings Excellent communication skills for working with children, seniors, individuals with disabilities or behavioral health needs; capable of working within multidisciplinary teams in inpatient or outpatient environments Join us to be part of a vibrant team dedicated to delivering exceptional patient-centered care.
