In this vital role, you will provide personalized care to individuals across various settings, including assisted living communities, nursing homes, and private homes. Your energy and attention to detail will help promote independence, comfort, and well-being for clients with diverse needs. Responsibilities Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other equipment Monitor and record vital signs accurately while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality Observe patients closely for changes in condition, including signs of dementia, Alzheimer's disease, or other memory impairments, and report findings promptly Administer medications safely following prescribed protocols and document all medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist with specialized care needs such as catheter care, hospice support, dementia care, and Alzheimer's care to enhance quality of life Support patients with nutritional needs by preparing meals tailored to dietary restrictions and assisting with feeding when necessary Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and ensuring infection control standards are met Experience Prior experience working in long-term care facilities like nursing homes or assisted living communities is highly valued Demonstrated ability to work effectively with individuals with developmental disabilities or disabilities requiring specialized care Familiarity with medical terminology, medical documentation practices, and HIPAA compliance is essential Hands-on experience with home health services including home & community care is preferred Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia-specific certifications are a plus Proven ability to operate EMR/EHR systems efficiently for accurate record keeping and communication Strong caregiving skills complemented by patience, empathy, and excellent communication abilities Join us in providing exceptional care that transforms lives!
