In this vital role, you will provide essential support to nursing staff and deliver high-quality patient care across diverse settings, including long-term care facilities, hospitals, assisted living communities, and home care environments. Your energy and commitment will help foster a safe, comfortable, and nurturing environment for individuals of all ages and needs. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices. Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature, ensuring accurate documentation in Electronic Health Record (EHR) systems like Epic or Cerner. Support medication administration under supervision, following strict aseptic techniques and HIPAA regulations to protect patient confidentiality. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral health support. Collaborate with healthcare teams in acute care settings such as ICU or Level I Trauma Centers, contributing to patient safety during complex medical procedures. Assist with medical procedures such as catheter care, phlebotomy preparation, and wound care in various clinical environments including outpatient clinics and hospital units. Maintain a clean and safe environment by adhering to infection control protocols, practicing aseptic techniques, and ensuring proper use of medical supplies. Support patients in rehabilitation or post-acute care settings by assisting with mobility exercises and ensuring comfort during recovery processes. Skills Proven experience working with diverse populations including seniors, pediatrics, individuals with disabilities or developmental disabilities, and those requiring hospice or palliative care. Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or similar platforms for accurate medical documentation. Strong knowledge of physiology, anatomy, medical terminology, and basic life support (BLS) procedures to ensure safe patient care. Experience in various healthcare environments including hospitals (ICU, trauma centers), nursing homes, assisted living facilities, outpatient clinics, urgent care centers, and home & community care settings. Ability to perform complex patient service tasks such as vital signs monitoring, medication administration, catheter care, and infection control practices. Skilled in working with individuals with behavioral health needs or dementia-related conditions using specialized memory care techniques. Excellent communication skills for effective collaboration with multidisciplinary teams while maintaining patient privacy under HIPAA guidelines. Capable of handling emergency situations calmly in high-pressure environments like Level II Trauma Centers or emergency medicine units.
