This vital role involves providing personalized care to individuals in residential support. You will be a key supporter in promoting independence, dignity, and comfort for our clients while working in a dynamic and supportive atmosphere. All positions are paid, recognizing the importance of your contributions from day one. Responsibilities Deliver direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure patient stability. Observe patients closely for changes in condition, including memory or behavioral shifts related to dementia or Alzheimer's disease, and report findings accurately. Administer medications and treatments following medical documentation standards while maintaining compliance with HIPAA regulations to protect patient privacy. Support patients with medical procedures such as catheter care, hospice needs, and assisting with medical records management using Electronic Health Record (EHR) systems. Assist with meal preparation and feeding, ensuring dietary restrictions are followed and nutritional needs are met. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry and general cleaning to promote hygiene and comfort. Collaborate effectively with healthcare teams by documenting care activities in EMR systems and participating in care planning discussions. Requirements Valid CNA certification or equivalent licensure required; prior experience in long-term care or nursing home environments is highly preferred. Demonstrated experience working with individuals with developmental disabilities or dementia care needs. Knowledge of medical terminology, medication administration protocols, and vital signs measurement. Certifications in CPR (Cardiopulmonary Resuscitation) and first aid are essential; additional certifications such as hospice care or catheter care are advantageous. Experience utilizing EMR/EHR systems for accurate medical documentation; familiarity with HIPAA regulations is mandatory. Ability to lift patients safely using assistive devices like Hoyer lifts; physical stamina to perform caregiving duties throughout shifts. Strong communication skills to interact empathetically with patients, families, and healthcare teams effectively.
