This role focuses on helping clients maintain their comfort, independence, and overall well-being through personalized assistance with daily activities. Primary Duties Working under the direction of a Nurse Manager and according to each client's service plan, responsibilities may include: Providing companionship and meaningful social interaction. Assisting with ambulation, transfers, repositioning, and wheelchair support. Supporting meal planning, grocery shopping, meal preparation, and nutritional needs. Assisting with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene. Completing light household tasks such as laundry, organizing, and basic housekeeping. Offering medication reminders and encouraging compliance with prescribed routines. Observing and reporting changes in a client's condition, including tracking intake and output when required. Escorting clients to appointments, community activities, and essential errands. Documentation & Reporting Complete visit notes and service records through the agency's electronic documentation platform. Communicate any concerns related to a client's health, safety, or behavior to supervisory staff promptly. Attend mandatory training programs and staff meetings. Fulfill annual continuing education requirements. Candidate Profile Successful applicants are dependable, caring, and committed to providing respectful, client-focused care. Desired Experience & Skills Previous experience providing personal care or assistance to adults and seniors. Experience supporting individuals with dementia, Alzheimer's disease, or memory-related conditions is a plus. Ability to use smartphones and basic technology for scheduling, communication, and documentation. Knowledge of applicable healthcare standards and regulations. Strong verbal and written communication abilities. Commitment to maintaining confidentiality and professional boundaries. Work Conditions Services are delivered in private residences and residential care settings. This position requires physical activity, including standing, walking, bending, assisting with mobility, and lifting or moving up to 25 pounds when necessary. This is an on-site caregiving role and is not eligible for remote work. Minimum Requirements Must be 18 years of age or older. High school diploma, GED, or equivalent. Active PCA certification or current CNA credential. One year of caregiving or direct care experience preferred. Valid driver's license, dependable transportation, active automobile insurance, and current negative TB/PPD screening.
