Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates accurately in electronic health record (EHR) or electronic medical record (EMR) systems Observe patients closely for changes in condition, including patient observation for signs of dementia, Alzheimer's disease, or other memory impairments Administer medications and perform basic medical procedures such as catheter care and wound dressing under supervision Support patients with medication administration while adhering to HIPAA regulations and medical documentation standards Assist with meal preparation, feeding, laundry, cleaning, and maintaining a safe environment for residents or clients Document all patient interactions thoroughly in medical records and ensure compliance with privacy regulations like HIPAA Skills Strong knowledge of medical terminology and procedures related to senior care, hospice care, dementia care, and memory care Experience working with individuals with developmental disabilities or disabilities requiring specialized attention Proficiency with EMR/EHR systems for accurate documentation of patient data and medical records Ability to perform patient observation effectively and recognize signs of distress or deterioration Skilled in assisting with ADLs, including bathing, dressing, mobility support, catheter care, and medication administration Knowledge of hospice care practices and working with patients at end-of-life stages Competency in vital signs measurement and basic medical procedures such as CPR certification and medication management Experience in home & community care environments along with long-term care settings is highly desirable Excellent caregiving skills combined with a compassionate approach to patient-centered care Ability to work independently or as part of a team in live-in employment or shift-based roles Join us if you're passionate about providing high-quality care that enhances the lives of others.
