  • , you will provide compassionate and skilled care to our clients in their homes, supporting their physical, emotional, and social needs. You will work closely with our clinical team to develop and implement personalized care plans, ensuring our clients receive the best possible care.
  • Responsibilities:
  • Provide direct care to clients, including personal care, transfers, and ambulation
  • Assist clients with activities of daily living (ADLs), such as bathing, dressing, and grooming
  • Administer medications and treatments as directed by the client's care plan
  • Monitor vital signs and report any changes or concerns to the clinical team
  • Assist with meal preparation and feeding, as needed
  • Engage in activities that promote socialization, mental stimulation, and emotional well-being
  • Document client care and progress, maintaining accurate and thorough records
