Job Description

Company Overview Community Health Services Corporation is dedicated to providing exceptional care to the residents of Prairie du Chien and surrounding areas. As the only skilled nursing care corporation in Crawford County, we focus on leading by example and making a positive impact in our diverse community through our comprehensive healthcare services. Base wage (without shift differentials): $19.70 - 22.10/hour + 3rd
Shift Differential:
$5.25/hour +
Weekend Shift Differential:
$4.50/hour 10pm - 6am; every other weekend and holiday. Summary As a Nursing Assistant at Community Health Services Campus, you will play a vital role in delivering compassionate and high-quality care to our residents. This position is essential for maintaining the well-being of our patients and supporting their daily activities within our skilled nursing facilities. Responsibilities Assist residents with daily living activities such as bathing, dressing, and grooming. Monitor and record vital signs, reporting any changes to nursing staff. Provide catheter care and support for residents with specific medical needs. Support residents with dementia through compassionate engagement and assistance. Maintain a clean and safe environment for residents at all times. Assist in mobility and positioning of residents to ensure comfort and safety. Communicate effectively with team members to coordinate patient care. Participate in training sessions to enhance skills and knowledge related to patient care. Qualifications Certified Nursing Assistant (CNA) certification is required. Experience in skilled nursing or assisted living environments is preferred. Knowledge of anatomy and basic medical terminology is essential. Strong interpersonal skills with the ability to connect with diverse populations. Compassionate approach towards caregiving, especially for individuals with dementia. Ability to work collaboratively within a healthcare team. Basic understanding of emergency procedures is beneficial but not required.
