Job Description

General Statement of Duties:
The Certified Nursing Assistant (CNA) provides personal care to hospice patients. Provides services to assist in the maintenance of a safe, clean environment.
Level of Responsibility:
Reports to Nurse Manager, Potomac Team (primarily Berkeley County, WV)
Status:
Non-exempt, hourly
Pay Range:
Starting at $20.50/hour; eligible for night differential $2 (Monday-Friday, 8 pm - 8 am), and weekend differential $3 (Saturday and Sunday, any times)
Schedule:
Full Time; Monday-Friday, 8 am - 4:30 pm; participates in back-up rotation
Duties and Responsibilities:
•Denotes essential job functions. Provides personal care to patients including but not limited to bathing, oral hygiene, dressing, nail, skin and hair care, to maximize comfort in accordance with the aide plan of care and the patient/family wishes. Transfers patient from bed to chair, bed to commode, chair/commode to bed and other transfers. Transfers patient by means of Hoyer Lift. Provides and assists patient with ambulation, exercises, and assists with use of walker or cane. Meal preparation (in keeping with diet restrictions) for patient and related tasks such as feeding patient and washing dishes. May assist with meal preparation at the inpatient facility on an as needed basis. Reports significant changes in patient's condition to Nurse Manager and/or Hospice Nurse for appropriate follow-up. Establishes and maintains good working relationships with the patient and family, the attending physician, coworkers and outside agencies. Provides temporary respite to a patient's family. When working in a facility, complies with facility-specific requests for reporting, documentation, PPE use, and other regulatory and facility requirements. Reports abuse and neglect to the appropriate authority following organization policy. Assists with staffing needs at the Inpatient Facility if needed. Assists in maintaining clean, safe, comfortable environment for patient. Cleans patient equipment as needed (ex. potty chair). Performs routine housekeeping tasks as it relates to maintenance of a safe, healthy environment which may include: household maintenance and cleaning, laundry care and changing linens, meal preparation, and related errands. May assist with housekeeping at the inpatient facility on an as needed basis. Records interventions on paper and electronically in accurate manner and submits within documentation guidelines. Participates in back up call coverage for unexpected absences or other needs as determined by the Staffing manager at the inpatient facility as needed. Completes 12 hours of approved in-service education per year. Demonstrates competency as hospice aide. Meets visit average standards established by the organization. Ensures aide supervisory is completed by the RN at least once every 28 days. Assists with answering phones/making calls as needed. Complies with the employee standards listed in the general evaluation for hospice employees. Performs other duties as assigned. Essential job function
Education & Experience:
Certified Nursing Assistant with current WV license is required. Minimum of one year experience as nursing assistant/home health aide. Provides own transportation, and a license to drive with current liability insurance. Driving record meets Hospice's insurance company standards. Physical Qualifications Ability to transport self to office and patient care settings throughout the service area. Ability to walk from car to patient's room in their care setting and perform essential job functions. Ability to lift and move patients and equipment. Ability to communicate verbally and in writing in English language. Ability to hear normal conversation in person and via phone. Ability to read normal or condensed print.
