The Team Leader will have a passion for delivering the highest quality care to our patients. Additionally, the Team Leader will have solid leadership skills, with the ability to establish positive and collaborative relationships built on trust and respect. This position is primarily responsible for assisting and providing safe, consistent, and appropriate patient care within his or her scope of practice. Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. RN/LPN's role will include but not limited to: the assessment, planning, implementation, and evaluation of patients' statues, plan of care, and goal achievement, as well as supporting the RN Manager in the day to day operations of the clinic.
Entering information into Electronic Medical Record (EHR).Scheduling appointments, keeping medical record current, and working with insurance companies.
Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments.
Assists in preparing and directing patients for examination to meet SVMS objectives.
Performs effective in-take assessments including a brief patient history, chief complaint, current medications, allergies, and significant previous medical/psychosocial problems.
Screens assigned telephone messages and processes inquiries, or refer to appropriate medical personnel per SVMS protocols. Effectively performs specific laboratory tests/Point of Care testing that has been approved by the Laboratory Director. Performs nursing procedures and treatments, and medication administration according to SVMS protocols upon request of the provider. Notifies the physician of significant findings or changes in patients physical or mental condition. Maintains a thorough knowledge of special programs offered by SVMS including family planning, EPSDT and cancer control, and of eligibility and guidelines of these programs which are available to patientsDemonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
Demonstrates courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff.
Maintains patient confidentiality and privacy at all times according to SVMS policies and procedures; uses discretion when discussing patient matters.
Acts as role model for ancillary personnelPractices within the limits of the Nurse Practice Act and understands legal consequences of nursing action. Other duties as assigned
Must have a current nursing license in each state of desired practice. Must have the demonstrated ability to function effectively within a multi-disciplinary team. Knowledge of lab testing, diagnostic equipment, and medical procedures and protocols is required. Must have excellent skills in patient communication and education. Annual basic BLS/CPR certification is also required. Previous experience working as an RN is preferred.
SPECIFIC DUTIES AND RESPONSIBILITIES
As an Registered Nurse, you will perform your job duties according to the Tate Nursing Practice Act while upholding the utmost ethical and safe techniques always keeping the care of the patient as your top priority. Promotes patient independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions. Gives medication as directed by the provider, vital signs; such as blood pressure, temperature, and weight, basic wound care, injections, and immunizations.Entering information into Electronic Medical Record (EHR).Scheduling appointments, keeping medical record current, and working with insurance companies.

Qualifications:
Must be a graduate of an accredited College of Nursing School.Must have a current nursing license in each state of desired practice. Must have the demonstrated ability to function effectively within a multi-disciplinary team. Knowledge of lab testing, diagnostic equipment, and medical procedures and protocols is required. Must have excellent skills in patient communication and education. Annual basic BLS/CPR certification is also required. Previous experience working as an RN is preferred.
