Part-time, Full-time TITLE:
HOSPICE AIDE & CNA
  • Community Program

NURSE, CLINICAL DIRECTOR DESCRIPTION
As a Hospice Aide & CNA with Hospice of Laramie, you make a lasting impact that betters the lives of our patients AND their families. You will provide excellent care and support to hospice patients and team members in their homes, designated facilities, and/or in the Hospice House in accordance with the Hospice Philosophy. We are looking for professional and compassionate caregivers to be a part of our amazing team here at Hospice of Laramie.

RESPONSIBILITIES
:
  • Patient Contact
  • participating in CNA standards of practice including obtaining vital signs, assisting with hands-on personal care such as feeding, bathing, mouth care, transfers, etc. Visiting with patients and family to provide a positive, dignified, and respectful end of life.
  • Confidentiality
  • Maintain confidentiality concerning all patient records and/or patient and family/caregiver interactions to the standard of HIPAA.
  • Meal Preparation & Clean-up
  • Preparation of meals based on desires of patients & families
  • Clean & Healthy Living
  • Maintain a clean and healthy-living patient care environment; change linens and patient room housekeeping; stock cabinets and supplies.
  • Communication
  • Report changes in patients' needs to other nursing staff. Maintain regular communication with RN on Duty and/or Clinical Director. Communicate instructions to patients, family members, and/or caregivers.
  • Trainings
  • Demonstrate continued professional growth and development through participation in educational. programs, continuing educational courses, and in-service training. Assists in the orientation process and continuing education opportunities for Care Givers and CNAs.
  • Record Keeping
  • Record in a timely manner all activities in the chosen Electronic Health Record charting database. If applicable, participate as a member of the IDT and attends regular meetings.
  • Positivity
  • Work to improve services and provide positive and appropriate care for the terminally ill and their families.
  • Initiative
  • Ability to work alone, finding helpful tasks to assist with the above areas of hospice.
QUALIFICATIONS
  • REQUIRED:
    Knowledge of hospice services and end-of-life stages Effective Communication Skills Computer proficiency Executes instructions with competence Initiative and independence to find additional tasks helpful to the goals of hospice care and the Hospice of Laramie philosophy Ability to follow established protocol; maintain quality and safety Good judgement
  • ability to make effective decisions in critical situations Have physical endurance for shifts predominantly standing, moving, bending, and lifting.
(Move up to 50 lbs. safely.)
QUALIFICATIONS
PREFERRED:
Knowledge of medical terms, applicable medical equipment, patient evaluation, and triage procedures. Experience with patient care charting, record keeping, medical computer applications, and procedures. Knowledge of patient evaluation and triage procedures. Ability to set, maintain, and achieve effective goals. Previous caregiving experience or healthcare experience.
OTHER REQUIREMENTS
Pass background check and drug test Valid licensure as a Certified Nursing Assistant (CNA) in the State of Wyoming (Home Health Aide Competency Certification completion (notarized)) Valid CPR (cardiopulmonary resuscitation) certification preferred. Valid driver's license and current proof of insurance. Completes 6 to 10 hours of in-service/online training every twelve (12) months (Relias).
WORKING CONDITIONS
Locations may include: Patient homes, designated facilities, and Hospice House. Regularly exposure to biological and chemical hazards, fumes, vapors, infectious diseases, noise, and confined spaces. May be on feet for extended periods of time. Employees may be required/asked to perform duties/tasks outside those listed in the job description. Fair Labor Standards Act is designated by position.
