Lead Care Manager - Butte CARES Community Action Agency of Butte County, Inc. - 2.9 Chico, CA Job Details Full-time $24 - $28 an hour 2 hours ago Benefits Health insurance Dental insurance Flexible spending account Paid time off Employee assistance program Vision insurance 403(b) Life insurance Qualifications MCP Social service program staff supervision Employee relationship building Driver's License Data management Case management Driving Motivational interviewing Health information management Care plans Full Job Description Under the supervision of the Program Supervisor, this position will require outreach, case management, tenant screening, assessments, and client advocacy, assisting members who are Medi-Cal eligible. This position will also require the ability to track, document and report data. This position is a split between healthcare management and street outreach. A higher percentage of this position will be managing the case load with Medi-Cal members. This position will require 80% case management and 20% outreach.
ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES
  • Coordinate with individuals and entities to ensure a seamless experience for members and avoid duplication of services.
  • Engage eligible members and oversee the provision of Enhanced Care Management (ECM) services, including completing assessments, developing care plans, and implementing care plans.
  • Offer services where the member lives, seeks care, or finds most easily accessible, in accordance with managed care plan (MCP) guidelines.
  • Connect members to other social services and supports they may need, including transportation and Community Supports.
  • Advocate on behalf of members with healthcare professionals.
  • Utilize motivational interviewing, trauma-informed care, and harm-reduction approaches.
  • Coordinate with hospital staff on discharge plans.
  • Accompany members to office visits as needed and according to MCP guidelines.
  • Monitor treatment adherence, including medication.
  • Provide health promotion and self-management training.
PHYSICAL REQUIREMENTS & WORKING CONDITIONS
Sit for extended periods; frequently stand and walk; normal manual dexterity and eye-hand coordination; lift and move objects weighing up to 25 lbs; corrected hearing and vision to normal range; verbal communication; use of office equipment, including computer, telephone, calculator, copiers, and FAX. Occasional driving to attend meetings. Requirements are representative of minimum levels of knowledge, skills, and experience required. To perform this job successfully, the worker must possess the abilities and aptitudes to perform each duty proficiently Work is performed in office, community, and community shelter environments; work is performed with people who may exhibit unstable and potentially violent behavior; continuous contact with clients, the general public, and other staff. Some evening and weekend hours will be required.

Skills:
  • Organize, develop, and deliver the services and program of the Healthy Home Program.
  • Ability to lead and guide staff.

  • Effectively communicate both orally and in writing.
  • Understand the needs of the transient population and relate positively to those needs.
  • Communicate tactfully and effectively with Members.
  • Maintain the security and confidentiality of specified information and records.
  • Develop, prepare, and maintain records and reports.
  • Establish and maintain cooperative working relationships.
  • Maintain and observe all safety precautions and practices.

Training & Experience:
A typical way to obtain the required knowledge and abilities would be:
  • Bachelor of Arts Degree; AND
  • A minimum of two (2) years of case management experience in a related area; OR
  • Any combination of training and experience which would likely provide the required knowledge and abilities is qualifying.
Special Requirements:
  • Possession of a valid California Driver's License with a good driving record.
  • Pass a post-offer / pre-employment TB clearance renewable annually.
  • Pass a post job-offer / pre-employment background check and Live Scan clearance.
