Job Description

Position Summary The Care Manager plays a key role in supporting elderly clients and their families by coordinating individualized care, anticipating evolving needs, and helping ensure a safe, compassionate care experience. This position serves as a bridge between clients, families, caregivers, and community resources to promote continuity of care and enhance quality of life. The Case Manager is responsible for care planning, ongoing case management, caregiver supervision, and connecting clients with appropriate outside agencies and services to support comprehensive care. This role also helps introduce prospective clients and families to Well Being's services and supports them in understanding care options available to meet their needs. Key Responsibilities Client Care Coordination & Case Management Conduct initial and ongoing client assessments to identify care needs, preferences, and concerns Develop, implement, and regularly update individualized care plans Monitor client well-being and proactively anticipate changing needs Maintain accurate and timely case notes and documentation Collaborate with internal team members to ensure continuity and consistency of care Family & Client Communication Build strong, trusting relationships with clients and family members Serve as a primary point of contact for care-related questions and updates Communicate changes in condition, care concerns, or recommendations promptly and professionally Provide support and guidance to families navigating care decisions New Client Consultations & Service Education Meet with prospective clients and families to assess care needs and discuss available support options Educate potential clients on services provided by Well Being and how those services can be personalized to support safety, independence, and quality of life Answer questions regarding care options, caregiver support, scheduling, and next steps for services Help families feel informed, supported, and confident as they explore care solutions Communicate potential client needs with the office team to support a smooth onboarding process Resource Coordination & Referrals Identify additional support needs and connect clients/families with appropriate community agencies, healthcare providers, or outside resources Coordinate referrals to ensure access to comprehensive care and supportive services Follow up on referrals and advocate for timely services when needed Caregiver Supervision & Support Supervise caregivers in the field to ensure care plans are followed and clients' needs are met Provide coaching, guidance, and support to caregivers Address performance concerns or care-related issues promptly Participate in caregiver check-ins and help maintain a high standard of compassionate care Qualifications Experience in case management, senior care, social services, healthcare, or related field preferred Strong understanding of elderly care needs and community resources Excellent communication and relationship-building skills Ability to assess needs, problem-solve, and respond proactively Comfortable meeting with prospective clients and discussing care options with professionalism and empathy Strong organizational skills and attention to detail Ability to maintain professionalism and confidentiality Valid driver's license and reliable transportation for client visits Preferred Qualities Compassionate and client-centered Strong critical thinking and ability to anticipate needs Calm and professional in changing situations Team-oriented with the ability to work independently Passion for supporting seniors and helping families feel informed and cared for At Well Being, we believe caring for seniors is both a responsibility and a privilege. Pay:
$20.00 - $25.00 per hour
