Qualifications:
Bachelor's degree in social work, criminology, sociology, child development or related field, or year-for-year experience working with systems-impacted youth and/or their families, with 2 years of Case Management experience preferred. Background Clearance Requirement:
Employment will be contingent on successfully passing a mandatory background check through Fresno County Probation. This will be completed, passed and clearance will be maintained and renewed annually according to the satisfaction of Probation. An outside background clearance will not be accepted or considered. Experience:
Working with diverse and underserved populations is essential. This may include individuals impacted by the juvenile justice system, formerly incarcerated adults, at-risk youth, or clients facing barriers such as homelessness, substance use, or limited educational attainment. Such experience demonstrates the ability to apply trauma-informed and culturally responsive practices when supporting clients in achieving employment goals. Applicants should possess experience in facilitating reproductive health education and family advocacy. This includes facilitating reproductive health education classes for young adults who are in custody and out of custody. This will also include the facilitation of family reunification services in partnership with the Fresno County Probation Department. Description of Duties and Responsibilities The Family Advocate and Care Manager delivers health education and care management services designed to support clients in achieving long-term stability, self-sufficiency, and overall well-being. This role combines reproductive health, family reunification, and coordinated care services to ensure clients receive comprehensive, client-centered support. The Family Advocate and Care Manager provides case management and advocacy services . This includes connecting families to community resources, assisting with applications for benefits or services, coordinating referrals, and monitoring progress toward established goals. Family Advocates often serve as liaisons between families and external systems, including schools, healthcare providers, social service agencies, and juvenile justice organizations. As part of a multidisciplinary Care Team within the Enhanced Care Management (ECM) Program, the Family Advocate and Care Manager also coordinates care and supportive services. This includes managing assigned cases, facilitating access to healthcare and community resources, coordinating benefits, and providing education to help clients navigate systems of care effectively. The Family Advocate and Care Manager collaborates with clients, families, caregivers, and service providers to promote wellness, resilience, and independence. Serving as both an advocate and liaison, this role ensures seamless coordination across employment, health, and social service systems to maximize client outcomes and support long-term success. Supervision:
The Family Advocate and Care Manager reports to the SYTF Coordinator Major Responsibilities Assess client needs in the areas of physical health; mental health; SUD; oral health; trauma- informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports. Oversees the development and implementation of the Individual Care Plan/Health Action Plan Connect clients to other social services and supports that are needed (e.g., community support group). Establishing and maintaining liaison with community organizations, local entities, and community stakeholders for outreach and engagement. Outreach activities to reach participants who are under- resourced and/ or underserved. Attend relevant ECM unit and agency meetings, trainings, events, activities and assignments. Perform other related duties as assigned by the ECM Services Unit Manager/Director Assist program coordinators to develop client files and case plans for participants for a successful transition of the participants from juvenile incarceration to least restrictive programs including independent living programs, employment and education programs and homes in the community. Provide support for family reunification, co-parenting, and family planning to youth who are parents. Provide reproductive health education Provide ongoing case management, various screenings, and referrals Conduct assessments Assist with maintaining Focus Forward database entering client information and documenting all services and client outcomes. Attend and participate in collaborative meetings (CFT/MDTs) Remain flexible regarding schedule (evenings and weekends are required and based on program/participant needs) Attend relevant training, workshops, and community meetings and events. Specific/Additional Duties:
Be responsive, consistent, and reliable with all clients, staff, and partners. Maintain up-to-date, well-organized, and accurate case files, records, and databases. Facilitate various groups such as support groups, independent living skills workshops, etc. Ability to report out on all assessments and case management updates, challenges, or concerns. All other duties as assigned Required Knowledge/Skills Able to effectively communicate the benefits or program enrollment Excellent verbal and written communication skills and organizational skills Demonstrated computer skills including, but not limited to using databases, use of Microsoft Office, Social Media, Website and Email Demonstrated Customer Service Skills Ability to develop and maintain professional working relationships with management, co-workers, partners, and the public. Ability to work under pressure and handle multiple tasks simultaneously. Knowledge of available resources within the community for the client population Ability to work with youth, young adults and families who face multiple risk factors, barriers and have had justice and/or child welfare involvement Ability to speak with clients one on one and to speak to large groups of people which may include public presentations and facilitating workshops Ability to think critically and identify opportunities, challenges, needs and prioritization of work to meet mandatory outcomes of program and organization Must exercise good judgment, be a positive role model, and maintain professionalism, especially as it relates to boundaries with participants Must be organized and be able to connect program and client needs to other services, may need to think 'outside the box' and brainstorm with staff and partners to fully meet client and family needs Have a basic understanding of the child welfare and justice systems and ability to learn to navigate both systems on behalf of youth Bi-lingual a plus Work Environment and Physical Conditions/Demands:
Sitting/standing for prolonged periods of time, lifting to approximately 40 pounds (more or less as needed), some meetings and appointments or presentations offsite in the community, frequent computer and telephone usage, physical coordination, working in noisy or crowded environment, speak loudly, stopping, bending, lifting, reaching, must drive vehicle. Must be comfortable interacting with clients facing various challenges and provide services in a non-judgmental manner Job Type:
Full-time, 40 hours per week, non-exempt Compensation:
$23.00 hourly. Benefit package that includes paid time off (sick leave, all Fresno County recognized holidays). Portion of medical insurance coverage; 100% dental (employee + 1 dependent), and vision coverage (employee only) and 403b retirement option with up to 4% employer match.
