Job Description

Memory Care Wellness Manager, LVN We are committed to fostering a supportive, collaborative community where team members bring professionalism, compassion, and integrity to every interaction. Key Responsibilities Clinical Oversight & Resident Care Oversee and support the delivery of resident care, including assessments, service plans, and progress documentation. Participate in resident move-in evaluations and ongoing assessments per Mosaic policies and state regulations. Review and document key resident updates daily in the community communication log. Attend and contribute to initial and ongoing service plan reviews. Review physician orders, MARs, and care documentation for accuracy and compliance. Ensure medication administration and care procedures align with regulatory and clinical best practices. Escalate unresolved clinical issues or medication discrepancies to the RN or Executive Director as needed. Medication & Health Systems Management Oversee the medication delivery system to ensure safety, accuracy, and compliance. Qualifications & Skills High school diploma or equivalent. Active LPN/LVN license in good standing per state regulations. (Required) Minimum 2 years of supervisory experience in senior living, assisted living, or healthcare preferred. Strong understanding of medication management, care planning, regulatory compliance, and best practices in senior care. Excellent leadership, coaching, and conflict-resolution abilities. Proficiency with EMRs, MARs, care documentation, and Microsoft Office Suite. Effective communication, adaptability, and commitment to resident-centered care.
