  • Full-time Overview Join our dynamic team as an Enhanced Care Manager/Community Supports Case Manager, where you will play a vital role in empowering individuals and families to achieve stability, wellness, and independence.
This energetic position involves coordinating comprehensive community-based services, providing crisis intervention, and supporting behavioral health needs. You will work closely with diverse populations, including those involved in child welfare, Medicaid, or Section 8 housing programs, ensuring they receive personalized support that fosters growth and resilience. Your proactive approach will help transform lives through compassionate guidance and strategic resource management. Duties Conduct thorough assessments to identify clients' needs across behavioral health, social services, and community supports. Develop individualized care plans that incorporate crisis management, addiction counseling, and motivational interviewing techniques. Provide crisis intervention during urgent situations to stabilize clients and connect them with appropriate emergency or mental health services. Coordinate with healthcare providers, social workers, childcare agencies, and workforce development programs to facilitate access to resources like Medicare or child welfare services. Supervise and mentor junior staff or volunteers involved in client support initiatives, fostering a collaborative team environment. Assist clients with navigating complex systems such as Section 8 housing or workforce development programs to promote self-sufficiency. Maintain detailed documentation of client interactions, service plans, and progress reports in compliance with organizational policies. Skills Strong expertise in crisis intervention and crisis management strategies to effectively de-escalate urgent situations. Proficiency in addiction counseling and behavioral health practices to support clients facing substance use challenges. Excellent communication skills with the ability to employ motivational interviewing techniques that inspire positive change. Experience supervising staff or volunteers within social work or community support settings. Knowledge of child welfare systems, childcare needs, and mentoring approaches to support vulnerable populations. Familiarity with government programs such as Medicare, Section 8 housing, and workforce development initiatives. Ability to handle sensitive information with confidentiality while demonstrating empathy and cultural competence.
