GED Driver's License Driving Full Job Description Hours :
40 hours per week; Monday - Friday, 8:00 am to 4:30 pm (no on-call, evenings, or weekends).
Pay Rate :
$24.90 to $26.21 per hour
Education :
High School/GED diploma with 2 years of experience working in social services or related setting required. Bachelor's degree in Public Health, Social Work, Psychology, Human Services or related field with 1 year of experience working in social services or related setting strongly preferred.

19 days of PTO and 12 paid holidays per year Free clinical supervision and CEUs Monthly contribution of $100 towards qualified student loan debt; access to Public Service Loan Forgiveness (PSLF) program for eligible individuals Choice of medical plans, with minimal employee cost/expenses and excellent coverage Several dental plan options; vision benefits Company-paid life insurance and Long-Term Disability Tax-Free Flexible Spending for medical/dental/childcare expenses Generous retirement plan contributions Free Employee Assistance Plan Employee discount program, including pet insurance plans Opportunities for advancement and growth As a Lead Care Manager I, you will : Provide mobile outreach and engagement Screen and assess for program eligibility Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals Connect clients and their families to a medical clinic and providers, as well as other clinic and community-based services Arrange transportation and accompany clients to office visits as needed Serve as an advocate for the client and family Use motivational interviewing, trauma-informed care, and harm-reduction practices Create and maintain all required documents in the electronic and paper record systems Additional Qualifications and Skills Experience providing case management services preferred Experience working with Seriously Mentally Ill (SMI) population is preferred Proficient computer skills, including MS Office This program is housed in a two-story building that does have an elevator; however, ability to navigate stairs may be required A valid California driver's license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position Ability to pass criminal/background clearance Healthy Connect San Diego is a great place to grow, come join our awesome team!
