Care Management Lead and Program Director Compensation:
$25 to $28 per hour Position Summary Located on the border between Los Angeles and Ventura counties in Westlake Village, the Enhanced Care Manager is a key member of Conejo Health's Enhanced Care Management (ECM) team. This role is responsible for managing a caseload of approximately 50 patients with complex medical, behavioral health, and substance use needs. The Enhanced Care Manager provides trauma-informed, person-centered, and community-based support, engaging patients in their homes, hospitals, and community settings. Working under the guidance of the Program Director, this position ensures consistent and high-quality delivery of ECM services.
KEY RESPONSIBILTIES
Direct Care Management Serve as the primary Enhanced Care Manager for assigned patients with complex health and social needs. Conduct outreach, home visits, and in-field engagement to build trust and support patient goals. Develop, implement, and update care plans in collaboration with patients and the care team. Coordinate referrals and access to services such as Medication Assisted Treatment (MAT), primary care, behavioral health, housing, transportation, and benefits. Documentation & Compliance Complete all required Electronic Health Records (EHR) documentation, including assessments, care plan updates, encounter notes, and follow-ups, in compliance with ECM standards. Ensure proper use of screening tools, patient tiering, and authorization workflows. Report clinical concerns, documentation issues, or service gaps to leadership promptly. Collaboration & Teamwork Participate in case conferences, interdisciplinary huddles, and team meetings. Communicate patient status changes and barriers to engagement in a timely manner. Seek guidance for high-risk or complex cases as needed. Program & Culture Support Model harm-reduction principles and trauma-informed care practices. Collaborate with peers and contribute to a positive, accountable team culture. Support equity, dignity, and compassion in all interactions with patients and colleagues.

Completes timely and accurate EHR entries.

Qualifications Education:
High School Diploma or equivalent required. College coursework in Social Work, Psychology, Addiction Studies, or Public Health preferred.
Experience:
Minimum 1 year required (2 years preferred) of experience working with individuals experiencing homelessness, behavioral health conditions, or substance use disorders. Previous care and case management experience preferred.
Certifications:
Certified Alcohol and Drug Counselor (CADC) or Certified Addiction Treatment Counselor (CATC) certification strongly preferred Community Health Worker (CHW) certification or equivalent experience preferred.
Knowledge & Skills:
Familiarity with California Advancing and Innovating Medi-Cal (CalAIM) ECM, Medi-Cal populations, and harm reduction principles strongly preferred. Proficiency with Electronic Health Records (EHR) systems, mobile work, and multidisciplinary teamwork required. Motivational Interviewing (working knowledge preferred).
