Ideally, seeking an experienced geriatric care manager who values holistic health and preventative medicine. This is not an entry level position. The Geriatric Care Manager plays a crucial role in providing comprehensive care and support to older adults and their families. This position involves coordinating medical services, facilitating communication with care teams, and ensuring that individuals are living with vitality and happiness. The ideal candidate will possess a strong background in communication, strong interpersonal skills and complex work environments. Our practice is the intersection of medicine, elder law issues, and complex family dynamics. This integrated care management position will provide comprehensive care coordination, resource navigation, and evidenced-based interventions for adults living in the Greater Denver Metro area. This position will work in close collaboration with medical teams, family members, care facilities and care partners. This position has significant growth potential. Why join us? Responsibilities Assess the medical, emotional, and psychosocial needs of older adults to develop personalized care plans. Ability to document accurate care notes. Coordinate with healthcare providers, including hospitals, Assisted Living facilities, Skilled Nursing and individuals aging in place at home. Monitor client progress and adjust care plans as necessary to meet changing needs. Provide education and support to clients and their families regarding health management and available resources. Collaborate with multidisciplinary teams to optimize individual outcomes through empathic listening, advanced problem solving and resource coordination. Advocate for older adults rights and preferences in all aspects of their care. Transport clients to appointments. Skills Strong background in social work, psychology, geriatrics, counseling and/or nursing. Proficient in planning processes and care management. Excellent communication skills for effective interaction with individuals, families, legal teams, healthcare professionals and care partners. Ability to perform thorough clients assessments and develop comprehensive care plans. Compassionate approach towards client care with a focus on enhancing quality of life for elderly individuals. Flexibility to address urgent or unexpected needs. Ability to work in a dynamic environment that includes seeing individuals in their homes or facilities in the Denver Metro area.
Preferred Qualifications:
Ideal candidates will have at least 5 years of experience in care management, integrated behavioral health in primary care, healthcare settings, protective services, victims services, or other relevant clinical experience. Experience or interest in an innovative agency advancing vitality and happiness throughout the lifespan with effective problem solving and effective team work. Experience working with older adults and knowledge of common behavioral and physical health issues among older adults (e.g., chronic disease management, sleep disruption, depression/anxiety, cognitive decline, grief, caregiver stress, etc.). This role requires a dedicated professional who is passionate about improving the lives of clients through effective care management and coordination of their emotional, physical, spiritual, and social needs.
