Job DescriptionLed by the community's Director of Health and Wellness, we are looking for compassionate and dedicated caregivers (Care Managers) to assist residents in all aspects of their daily life as indicated in the resident's care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.
Schedules:
2pm to 10pm: 32-37.5 hour position in Assisted Living and/or
Memory CareSalary Range:
$19.00 - $22.00 HourlyResponsibilities and DutiesAssist with self-administration of prescribed medication to resident and maintain related medical records, by way of queuing, prompting, and/or reminding; under the supervision of the RN and/or LPN.Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care planRespects the needs of team members and residents by adhering to schedule and documenting attendance properly.

Maintain the safety, comfort, dignity, and confidentiality of all residentsEncourages and escorts (when needed) residents to meals and activitiesEnsures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shiftsDocument all services provided in EHR for your assigned residents during your shiftMonitors and reports any change of condition in a resident immediately to your supervisorAppropriately utilizes and keeps electronic documentation "devices" safe and secure within the community; efficiently documents care/services provided during the shift.

Participates in and attends all in-service trainings as scheduledEnsures compliance with all Federal, State, and local regulations, as well as community policies and proceduresProvides basic housekeeping and/or laundry services when neededMay be asked to obtain weights and vitals of residents and document same per RN requestOther duties as assigned by supervisor, RN, and/or EDQualificationsHigh School Diploma or GED. At least 1 year resident care experience required.

Certified Nursing Assistant or Certified Home Health AidKnowledge and experience in working with the elderly population, preferred dementia experiencePhysical AbilitiesWhile performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.

An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
