Sunrise Senior Living, a leader in providing a nurturing environment tailored to the individual needs of each resident, is seeking a dedicated Care Manager to join our team. This position encompasses managing daily care and scheduled activities for our residents to ensure their health and well-being. It requires not only a thorough understanding of healthcare management but also a compassionate approach to handling the unique requirements of elderly individuals with various levels of dependency. The ideal candidate will contribute to developing personalized care plans, overseeing their execution, and ensuring that staff adhere to the highest standards of safety and care. Develop and implement personalized care plans for residents based on their individual health status and preferences. Monitor and evaluate the care delivered by nursing and support staff to ensure it meets company standards and regulations. Collaborate with a multidisciplinary team including healthcare professionals, social workers, and therapists to enhance the quality of life for residents. Provide regular updates to family members about the health and wellbeing of residents. Ensure compliance with all federal, state, and local regulatory requirements. Participate in the training and professional development of staff members. Assist in managing the budget related to resident care and monitor inventory of supplies. Strong leadership and organizational skills Excellent communication and interpersonal skills Ability to handle multiple tasks and prioritize effectively Comprehensive knowledge of healthcare regulations and elder care best practices Proficiency in medical software and record keeping Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Registered Nurse (RN) license is preferred. Certified in Geriatric Care Management or similar.
