Job Description

Job Summary We are seeking a compassionate and proactive Care Manager to join our dedicated team. In this vital role, you will coordinate and oversee personalized care plans for individuals with diverse needs, ensuring they receive comprehensive support that promotes their well-being, independence, and quality of life. Your energetic approach will empower clients to overcome challenges, navigate complex systems, and achieve positive outcomes. This position offers an exciting opportunity to make a meaningful difference in people's lives through collaborative care management, crisis intervention, and community engagement. Responsibilities Develop and implement individualized care plans tailored to each client's unique circumstances, including behavioral health, and social service needs. Conduct thorough assessments to identify clients' strengths, challenges, and goals, utilizing motivational interviewing techniques to foster engagement. Coordinate services across multiple agencies such as Medicaid ,SCOs Provide crisis intervention and crisis management support during urgent situations to ensure safety and stability for clients. Offer addiction counseling and behavioral health support to promote recovery and mental wellness. Supervise and mentor junior staff or interns, fostering a collaborative team environment focused on client-centered care. Maintain detailed documentation of client interactions, service plans, progress notes, and incident reports in compliance with regulatory standards. Requirements 1-2yrs of Proven experience in social work, behavioral health, or related fields with a strong background in crisis intervention and crisis management. Bachelors Degree Demonstrated expertise in addiction counseling, motivational interviewing, and child welfare practices. Supervising experience preferred; capable of mentoring team members effectively. Knowledge of housing programs such as Section 8 and familiarity with Medicare regulations is highly desirable. Strong understanding of community resources related to workforce development and healthcare services. Excellent communication skills with the ability to build trust and rapport with diverse populations. Ability to handle sensitive situations with professionalism, transparency, and empathy while managing multiple priorities efficiently. Join us in delivering impactful care that transforms lives! Pay:
$52,400.02 - $63,105.40 per year
