Campaign is a global movement of abolitionists creating emergency stabilization and aftercare programs that break the cycle of exploitation and help survivors find freedom from slavery. We are a faith-based nonprofit providing aftercare and residential programs for survivors of human trafficking. Since 2016, we've supported over 200+ women through crisis intervention and holistic recovery. Our mission is to empower survivors to heal, grow, and discover purpose through therapeutic care and community support. Rooted in the belief that "Hospitality Comes First," we foster a safe, dignified, and loving environment where survivors can stabilize and embrace their identity and calling.
OUR MISSION
We know that without safe housing, human trafficking survivors are re-victimized. HER Campaign provides aftercare programs that bridge the gap between rescue and freedom, cultivating spaces where holistic healing can take place.
PROGRAM PHILOSOPHY HER
Campaign utilizes a trauma-informed, faith-informed, and holistic recovery model that addresses the integration of mental, emotional, physical, and spiritual healing. Clinical services are delivered within ASAM Level 3.3 standards and are grounded in evidence-based treatment for substance use and co-occurring disorders. Participation in spiritual activities is voluntary and client-centered.
POSITION OVERVIEW
Services are delivered within an
ASAM 3.3
clinically managed residential framework with 24/7 structured staffing, interdisciplinary care coordination, and ongoing risk monitoring. The Care Manager provides individualized support and advocacy for residents recovering from substance use disorders, co-occurring behavioral health conditions, and those who have survived human trafficking. They work closely with each participant to assess needs, create personalized care plans, and connect them with holistic services such as trauma-informed therapy, medical care, education, life skills training, and community resources. Works within a 24/7 residential staffing environment with access to on-call clinical and supervisory support. The Care Manager ensures coordination of services, monitors progress, and advocates for residents' safety and long-term stability. They serve as a consistent point of support, fostering trust, empowerment, and hope while guiding residents toward healing and independence from intake through discharge and post-discharge coordination. Care Managers do not provide psychotherapy, diagnosis, or clinical treatment services and operate under clinical supervision.
KEY RESPONSIBILITIES
Intake & Assessment Assist with intake process and gather psychosocial information as directed by clinical staff Assist residents with Medicaid enrollment, eligibility verification, and benefit coordination within program timelines. Collaborate with clinical staff to address immediate medical, mental health, legal, and safety needs. Support development of individualized goal plans in coordination with clinical staff Individualized Planning & Goal Setting Provide case management services across the continuum of care from admission through discharge and coordinated post-discharge follow-up. Facilitate seamless transitions between levels of care, community providers, recovery supports, and other identified resources. Monitor progress toward individualized goals and assist residents in overcoming barriers to treatment engagement, recovery, and long-term stability. Support discharge planning and continuing care arrangements beginning at admission and continuing throughout treatment. Care planning is informed by ASAM multidimensional assessment (Dimensions 1-6), including ongoing review of biomedical, psychiatric, behavioral, and recovery environment needs. Establish short-term care management goals, including healthcare, Medicaid, and aftercare planning. Coordinate medical and legal supports based on identified need and program clinical guidance Support only essential legal interventions necessary for placement continuation or core documentation. Regularly review and update service plans based on progress and changing needs. Advocacy & Care Coordination Ensure consistent communication with the residential team. Serve as the central point of coordination between residents, therapists, healthcare providers, psychiatric/medical consultants (as applicable), and residential staff. Ability to engage respectfully with individuals from diverse backgrounds with trauma-informed, nonjudgmental care. Ability to maintain appropriate therapeutic boundaries and adhere to ethical service delivery standards. Documentation & Compliance Maintain accurate, timely, and confidential case notes, service plans, progress reports, incident reports, and exit summaries. Ensure compliance with organizational, ethical, and regulatory documentation standards. Maintain documentation in accordance with HIPAA standards and applicable state and program licensing requirements, supporting continuity within a clinical record system. Complete required documentation, service notes, and case management records within established organizational timelines. Ability to maintain extreme confidentiality to the organization, clients, and staff members. Crisis Management & Support Ability to recognize escalation cues and respond using trauma-informed de-escalation techniques and provide trauma-informed crisis intervention in alignment with de-escalation training. Collaborate with residential staff to ensure safety and de-escalation during incidents. Participate in needs assessments and exit planning when resident need exceeds program capacity. Conduct and document ongoing safety and risk screenings (including suicide, self-harm, and acute psychiatric decompensation risk) and escalate concerns per clinical protocol. Ability to identify safety risks, behavioral escalation, and changes in mental health status and escalate appropriately. Ability to manage exposure to trauma-related content and maintain professional functioning. Life Skills & Empowerment Support Guide residents in building independent living skills such as self-advocacy, time management, and communication. Encourage participation in therapeutic, educational, and resilience-focused activities. Medical Appointment Coordination Coordinate timely medical, psychiatric, and specialty appointments based on resident needs and clinical recommendations. Provide transportation for initial medical appointments; coordinate with Residential Coaches for telehealth, follow-ups, and transportation as needed. Collect and communicate medical updates promptly to ensure continuity of care. Transition & Discharge Planning Initiate transition planning in coordination with clinical recommendations by week 2 to prepare for the next phase of care. Coordinate with referral partners for placement, travel logistics, and documentation. Ensure warm handoffs to new programs with survivor consent and understanding. Provide post-exit follow-up with any outside resource referrals and client as appropriate. Resident Meeting Cadence Meet with each assigned resident at least twice weekly for ongoing support and stabilization. Maintain consistent appointment scheduling through Skylight (e.g., same day/time weekly) when possible. Clearly communicate and document any necessary schedule changes. Participates in interdisciplinary treatment team meetings and contributes information regarding resident progress, barriers to treatment, recovery environment concerns, and ASAM Dimensions.
EXPERIENCE/EDUCATION
High School diploma or GED required Associate or Bachelor's degree in human services, social work, psychology, or related field preferred Minimum 2-3 years of experience in behavioral health, case management, social services, residential care, or related human services environment Experience working with individuals with complex trauma, mental health needs, or substance use disorders strongly preferred Must be at least 18 years of age Ability to obtain and maintain required certifications and training within established timeframes Ability to work flexible hours including evenings, weekends, and holidays as required Employee must complete required onboarding and ongoing training as a condition of employment, including but not limited to: Trauma-informed care training and application in clinical practice Crisis intervention/de-escalation training (CPI or equivalent) CPR and First Aid certification (must remain current) Program-specific clinical, documentation, and compliance training Employee must also meet the following conditions of employment: Successful completion of all required background checks (including criminal background, registry checks, and exclusion list screening as applicable) Negative tuberculosis (TB) screening prior to or upon hire, with ongoing testing as required by program policy or public health guidance If assigned to duties involving client transportation or driving on behalf of the program, employees must possess a valid driver's license, maintain an acceptable driving record, and meet agency insurance requirements.

PHYSICAL DEMANDS
While performing the duties of this job, the employee is regularly required to travel from floor to floor and may be required to travel outside between buildings in varying outdoor weather conditions. The employee may also be regularly required to sit or stand for prolonged periods of time; regularly use hands to operate office machinery including, but not limited to, telephones, computers, fax, and photocopy machines; regularly walk, talk, and hear.
WORK ENVIRONMENT
While performing the duties of this position, employees may be exposed to emotionally intense or crisis situations. The work environment can at times be fast-paced, unpredictable, and may involve interactions with individuals experiencing trauma, distress, or behavioral escalations. Staff are expected to maintain professionalism, utilize de-escalation techniques, and support a culture of safety, empathy, and resilience. This role may require occasional travel, participation in community settings, or attendance at meetings outside of regular business hours. The noise level in the work environment is usually moderate to loud. The employee must occasionally lift and/or move up to 25 pounds. This position is subject to varying and unpredictable situations; may handle emergency or crisis situations; is subject to many interruptions; may handle multiple calls and inquiries simultaneously; and may occasionally handle absentee replacement on short notice.
