June 5, 2026 About the Role We're hiring a Care Manager to provide Tailored Care Management services to clients with complex behavioral health and healthcare needs in North Carolina. In this role, you'll support clients dealing with mental illness, substance use disorders, intellectual and developmental disabilities, and/or traumatic brain injuries. You will also address social determinants of health such as housing, employment, food insecurity, and finances, ensuring clients receive holistic care. As a Care Manager, you'll lead a team of Care Manager Extenders and oversee service provision to meet or exceed key performance indicators and client outcomes. This role includes up to 50% travel to support clients where they are in their journey. Responsibilities Deliver Tailored Care Management services to individuals with complex behavioral health and healthcare needs Lead a team of Care Manager Extenders to ensure service delivery aligns with client needs and desired outcomes Develop, implement, and monitor individualized care plans in collaboration with multidisciplinary teams Assess client progress and adjust care plans based on changing needs Connect clients to essential community resources such as housing, employment, and healthcare Maintain accurate documentation and ensure compliance with regulatory requirements Participate in ongoing training and professional development Required Qualifications Bachelor's degree in a related field (e.g., Health, Psychology, Sociology, Social Work, Nursing, or another relevant human services area) AND 2 years of experience working with the MH/SU/IDD
population OR Bachelor's degree in an unrelated field AND 4 years of experience working with the MH/SU/IDD
population OR Master's degree in a related field AND 1 year of experience working with the MH/SU/IDD
population OR Full or provisional licensure or certification with the appropriate North Carolina board (LCSW, LCMHC, LCAS, LPA, LMFT, etc.) OR Registered Nurse (RN) in North Carolina AND 4 years of experience working with the MH/SU/IDD
population Preferred Qualifications Experience in care management or case management Knowledge of social determinants of health and community-based resources Compensation & Benefits Full-time Employees:
