June 5, 2026 About the Role We're hiring a Care Manager Extender to support the delivery of Tailored Care Management services to individuals with complex behavioral health and healthcare needs in North Carolina. As a Care Manager Extender, you'll work alongside a Care Manager to assist clients in accessing essential services, manage appointments, and ensure effective care coordination. This role requires strong communication, organizational skills, and the ability to engage with clients in a supportive, compassionate manner. The position will require up to 75% travel. Responsibilities Assist Care Manager in delivering Tailored Care Management services Perform outreach, engagement, and follow-up with clients Coordinate appointments, arrange transportation, and provide reminders Facilitate health promotion activities and knowledge sharing Share client information with care managers and team members Track referrals and provide assistance in obtaining community-based resources Participate in case conferences and help assess unmet health-related needs Support Care Manager in ensuring clients have access to necessary social support services Required Qualifications High school diploma or equivalent Bachelor's Degree in social work, counseling, psychology, or nursing (preferred) At least two years of experience working with the

MH/SU/IDD

population Experience with outreach, engagement, and service coordination for Tailored Care Management One year of paid experience working directly with Tailored Care Management eligible population Lived experience with an I/DD or TBI, or as a Certified Peer Support Specialist Experience navigating the North Carolina Medicaid system
