The Supervisor ensures compliance with all care management standards, oversees quality improvement activities, monitors service delivery and documentation, and promotes positive outcomes for members with complex behavioral health, intellectual/developmental disability, traumatic brain injury, physical health, and social determinants of health needs. Essential Duties and Responsibilities Provide ongoing supervision and support to Tailored Care Managers in accordance with DHHS and Alliance Health requirements. Monitor compliance with care management standards, including assessments, care plans, member engagement, transitions of care, and documentation requirements. Review caseloads, productivity, outreach activities, and performance measures. Ensure timely completion of Comprehensive Care Management Assessments (CMCAs), Individual Care Plans (ICPs), and required updates. Monitor hospital admissions, discharges, emergency department utilization, and care transitions. Conduct quality assurance reviews and corrective action activities as needed. Provide coaching, training, and professional development to care management staff. Participate in interdisciplinary team meetings and coordination with providers, hospitals, schools, DSS, juvenile justice, and community partners. Ensure compliance with HIPAA, Medicaid, Alliance Health, DHHS, and agency policies. Prepare reports and performance data as requested. Support accreditation, audits, monitoring activities, and quality improvement initiatives. Minimum Qualifications Master's degree in Social Work, Human Services, Psychology, Counseling, Sociology, r related human services field. RN's qualify for this position. Qualified as a Care Manager Supervisor under North Carolina Tailored Care Management requirements. Minimum of three (3) years of experience working with individuals with behavioral health, I/DD, TBI, substance use, or complex medical needs. Minimum of one (1) year supervisory experience preferred. Knowledge of North Carolina Medicaid, Tailored Care Management, care coordination, and community-based service systems. Valid North Carolina driver's license and reliable transportation. Preferred Qualifications Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Registered Nurse (RN), or other relevant professional licensure. Experience with Alliance Health Tailored Plan. Experience supervising care management or case management staff. Experience utilizing electronic health records and population health management systems. Knowledge, Skills, and Abilities Strong leadership and supervisory skills. Excellent organizational and time management abilities. Ability to analyze data and implement performance improvement strategies. Knowledge of Medicaid managed care and Tailored Care Management requirements. Ability to work collaboratively with community partners and multidisciplinary teams. Excellent written and verbal communication skills. Physical Requirements Ability to travel throughout the service area. Ability to sit, stand, walk, and operate standard office equipment. Ability to attend community meetings and home visits as required.
Pay:
$28.00 - $32.00 per hour
