Full-Time About Healthcare of New York Agency Care Management Agency is committed to improving health outcomes for vulnerable populations by delivering coordinated, person-centered care. Through the Medicaid Health Home program, we provide comprehensive care management services that address the medical, behavioral health, and social service needs of individuals with chronic conditions. Position Summary Healthcare Network of New York is seeking a dedicated Care Manager to support Adult Medicaid recipients enrolled in the Medicaid Health Home program. The Care Manager will coordinate services across medical, behavioral health, and community-based providers to ensure members receive comprehensive, integrated care that improves health outcomes and quality of life. Key Responsibilities Conduct comprehensive assessments of members' medical, behavioral health, and social service needs. Develop and implement individualized, person-centered care plans in collaboration with members, families, and care teams. Coordinate care among primary care providers, specialists, behavioral health providers, hospitals, and community-based organizations. Assist members in accessing services such as housing support, transportation, food assistance, and other social determinants of health resources. Monitor member progress and update care plans as needed. Maintain accurate and timely documentation in accordance with Medicaid Health Home and agency standards. Conduct outreach and engagement activities with members in community settings, homes, hospitals, or provider offices. Participate in interdisciplinary case conferences and team meetings. Ensure compliance with Medicaid Health Home program requirements and agency policies. Qualifications Bachelor's degree in Social Work, Human Services, Nursing, Public Health, or a related field required. Minimum of 1-2 years of experience working with individuals with chronic medical, behavioral health, or substance use conditions preferred. Experience working with Medicaid populations or care management programs strongly preferred. Knowledge of community resources and social service systems in New York. Strong communication, organizational, and problem-solving skills. Ability to work independently and manage a caseload. Proficiency with electronic health records and care management platforms. Bilingual (Spanish or other languages) is a plus.
Pay:
$50,000.00 - $55,000.00 per year
