Care ManagerJob Summary:
CASES is seeking a Care Manager that will provide person-centered care management services to empower individuals to take control of their health and well-being. You will develop and implement individualized care plans, leveraging motivational interviewing techniques and care coordination to support members in accessing necessary services. With a focus on cultural sensitivity, trauma-informed care, and recovery-oriented practices, you will provide guidance and support to members and their families. This position is full-time Monday•Friday from 9:00 AM•5:00 PM.Salary:
The salary for this role is $50,000 per year.Location Address:
2090 Adam Clayton Powell Jr Boulevard, New York, NY 10027. Our office is easily accessible by public transportation.Workplace Flexibility:
Flexible•Staff have flexibility to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work. At least 80% fieldwork required.What You Will Be Doing:
Complete all documentation•e.g., assessments, plans of care, encounters•within required time frames in the web-based health home portal.Provide core care management services (Comprehensive Care Management, Care Coordination & Health Promotion, Comprehensive Transitional Care, Enrollee & Family Support, Referral to Community & Social Supports, Use of Health Information Technology (HIT) to Link Services).Provide care management services using motivational interviewing techniques to support consumers to address their problems by utilizing their strengths and abilities.
Assuring that member has access to engages in and retain needed services as defined in the members Care Plan; Services may include but not limited to: Acute, Primary, and Preventative Medical Care, Home Health Care, Chemical Dependency Services, Behavioral Health Services, Community social support, housing, state & federal entitlements. Meet required levels of case management contacts, a minimum of 4 per month per member, 2 of the 4 contacts being face to face with member in person.
Assist members by escorting them to appointments, e.g., medical, behavioral health, entitlements.
Involve family and significant others in the consumers care planning.
Respond to crisis situations as needed.
Use available databases and their reporting functions to conduct job duties, such as GSI Health Coordinator, ePACES/eMedNY, Health Commerce System Medicaid Analytics Performance Portal.
Keep abreast of current city, state, and federal regulations, laws, and initiatives pertaining to health home care management and Medicaid redesign; attend relevant training.
Provide all services in a culturally sensitive, trauma-informed, and recovery-oriented manner.
Other duties as defined by the team leader or CASES executive staff.
What We Are Looking For:
High School Diploma and CASAC certified; orBachelor's degree with 4 years of social services experience; orBachelors degree and a NYS teachers certificate; orBachelors degree and NYS licensure as a Registered Nurse; orMaster's degree with 2 years of social services experience2 years' experience in providing direct servicesStrong knowledge of wellness, recovery, and self-help.Effective communication and written skills are essential.
Proficiency with computer and databases.
Preferred but not required:
Bilingual skills in Spanish highly preferred.Employee Benefits:
