Job Description
Case Manager responsible for managing Managed Medicare (MMCR) and HMO residents in a Skilled Nursing Facility. Duties include submitting clinical documentation for initial and continued stay authorizations, tracking approvals and next review dates, coordinating with interdisciplinary teams (nursing, rehab, social work), issuing NOMNCs, and handling appeals to ensure appropriate level of care and reimbursement.
Shift & Schedule:
Full-TimeMonday- Friday9:00 AM
- 5:00 PMOccasional weekend coverage/rotation for time-sensitive tasks (e.g., NOMNCs, admissions) We offer competitive compensation packages including benefits such as:
