Job Description

Rehabilitation Support Services, Inc. is hiring a Care Manager for our new SOS-CTI Team. This position is a unique opportunity for experienced mental health professionals to work in one of the Governor's newly launched innovative Safe Options Support (SOS) teams, that will provide comprehensive care to individuals who are homeless.

The multi-disciplinary SOS team will consist of a Team leader, Licensed Clinicians, Care Managers, Peer Specialists. The team will support program participants in the community through the application of the highly acclaimed, Critical Time Intervention, evidence-based, model of care. Hours and Benefits:

Care Manager:
Bachelor's degree with at least 2 years experience in the behavioral health field preferred. Additional experience may substitute degree. Valid NYS Driver's License required. Responsibilities for the Care Manager include : Care Managers will be supervised by the Team Lead and work in collaboration with the Clinicians and Peer Specialists Care Managers will be responsible for working with individuals to develop person-centered care plans, link to providers and community resources, assist with housing applications and shelter placement, collaborate with providers, and maintain documentation. This position will be field-based. #HP123
