Job Description
Rehabilitation Support Services' Care Management Program currently has an opening for a Health Home Plus Care Manager located in Schenectady County. The schedule for this position is Monday through Friday 8am-4:30pm with some flexibility. The primary role of the Care Manager is to provide intensive Health Home services to a caseload of 15 individuals with the goal of reducing the rate of hospitalizations through engagement in all necessary community-based resources and services, and behavioral and physical health care. The Care Manager will work closely with community providers to ensure appropriate delivery of services to meet the needs of each participant. The Care Manager will conduct various assessments to assist in identifying client needs and create individualized care plans to reflect these needs.
