POSITION SUMMARY
The Care Manager will assure that Health Home enrollees receive needed medical, behavioral, and social services with the goal of improving individual health outcomes. Will have overall responsibility and accountability for coordinating all aspects of the individual's care. Will collaborate with multidisciplinary personnel (i.e. medical, mental health, nurses, etc.) to assure patients receive services as indicated in the enrollee's plan of care.
EDUCATION AND EXPERIENCE
Bachelor's degree in Health, Human Services, Social Work, Mental Health, or qualified field required or Associate's Degree in Health, Human Services, Social Work, Mental Health or certification as an RN or LPN and two years' qualifying experience.
PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds. Specific vision abilities required by the job include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
